Identifying Health Improvements Made Easy

The value of accurate information on safety and quality in health care for clinicians, policy makers and PHNs is certain. However, the availability of standardised measurement sets for reporting of major diseases across primary and secondary health care sectors remains fragmented.

With over ten years expertise in the sector, we have risen to this challenge. The Improvement Foundation works collaboratively with leading health care organisations to oversee the development and evolution of national quality and safety health care measurement sets. These nationally consistent measures are key to supporting front line teams to improve the delivery of effective, safe and efficient patient-centred care.

 

Working Collaboratively with PHNs

Our current measurement suite is extensive and, through collaboration, can be continually enhanced with additional data sets that are informed by requirements of peak health care bodies, including PHNs. To expand the measurement set,  an Expert Reference Panel is convened which brings together an appropriate mix of application and subject matter experts.

 

Get Involved
 

Peak bodies, including PHNs, are invited to get involved in expanding the national measurement set. By working together, we can identify measurement gaps and develop suitable indicators to measure the improvement of chronic diseases; improving the safety and quality of health care delivery.
